AGREE - We/I understanding that certain CACC activities, including but not limited to physical exercise and sports, by their very nature are involved with potential risk of physical injury. Relative to us/me, my son/daughter/ward as a participant of the CACC activities we/I join, we/I assume all risks and hereby release CACC, its co-sponsor, and instructors and leaders appointed by them from all liability by reason of any injury to any person or any damage to anything in connection with activity.
AGREE - We/I certify we/I as participant(s) of these activities are/am covered by medical insurance.
AGREE - We/I do hereby grant permission to medical aid or treatment for our/my son/daughter/ward/self as needed.

Description: CACC Delaware Table Tennis Club is a members' only club located at the Chinese American Community Center Montessori School. We meet Mon, Thu 7:30-9:30pm. Sat: 10:30 - 13:30 at the Gym. Annual membership fee is $125 a year.
